SDI's Agency Fee:
$1,400 - This fee includes non-medical prescreening and processing of the Donor, case management for the Donor and Recipient Couple, FDA medical screening of the Donor, semen analysis of the Donor and cryogenic storage.

Sperm Donor's Compensation:
$1,000 - $2,000 for sperm donor's compensation depending on their qualifications.

Psychological Evaluation & MMPI Test:
$450 - A Psychological Evaluation with an appointed Psychologist and administration of Donor's MMPI test.

Retrieval Fee:
$350 – This fee includes the cost for retrieving the samples, preparing 2 vials for shipping, filling the shipper with liquid nitrogen coolant, delivering it and returning it back to the lab.

Fees shown do not include Donor's medical screenings.
